Цель операции - профилактика ишемических инсультов. С 1980 года она стала самой частой процедурой, выполняемой в большинстве сосудистых центров. Но операции, выполняемые с целью профилактики нарушений мозгового кровообращения, сами могут быть прямой причиной развития неврологических нарушений (инсультов) у небольшой, но значимой группы пациентов. Операция каротидной эндартерэктомии является единственной формой механической реваскуляризации головного мозга с доказанной эффективностью, по сравнению с медикаментозной терапией, и низкой частотой периоперационных осложнений. Во многих исследованиях доказана целесообразность оперативного лечения, определены показания к выполнению КЭЭ у, так называемых, «симптомных» [1, 2] и «асимптомных» пациентов [3-5], а также разработана подробно сама хирургическая техника. Согласно исследованию NASCET [6], кумулятивный риск развития инсультов ко второму году при медикаментозной терапии составил 26% (из 331) в сравнении с 9% (из 328) у пациентов из группы хирургического лечения (уменьшение абсолютного риска: 17±3,5%, p0,001) (табл. 1). Частота обширных или фатальных ипсилатеральных инсультов в обеих группах составила, соответственно, 13,1% и 2,5%, (уменьше<br />ние абсолютного риска: 10,6±2,6%, p0,001), а частота инсульта/смертности 32% и 16%, соответственно (уменьшение абсолютного риска: 16,5±4,2%, p0,001). Полученные различия статистически значимы. Малое число осложнений обуславливалось низким риском при выполнении хирургического вмешательства. У пациентов из группы медикаментозной терапии риск осложнений/смертности возрастал параллельно степени выраженности сужения на каждые 10%, особенно при критических стенозах (70-99%). Данные результатов NASCET по «симптомным» пациентам продемонстрировало эффективность КЭЭ у больных мужского пола с умеренными стенозами сонных артерий (50-69%). Тем не менее, результаты у пациентов женского пола не были столь однозначны. Дополнительные данные были получены в европейском исследовании по хирургии сонных артерий European Carotid Surgery Trial (ECST) и исследовании ветеранов администрации Veterans’ Administration (VA) Symptomatic Carotid Stenosis Group.[1.
