The number of patients receiving surgery for a broken collar bone in the middle third are increased.. The number of complications grow parallel after surgery treatment. The purpose of our study To examine the number of complications arising after open reposition and osteosynthesis of fractures of the middle third of the clavicle during using different surgical access. Materials and methods We studied patients with a fracture of the middle third of the clavicle, who had surgical treatment with using a horizontal transclavicular access, horizontal access with release of the branches of supraclavicular nerve and miniinvasive vertical access, during their staying in the hospital and after 3, 6,12, and 24 months after surgery . Results and their discussion We have not observed complications such as pseudarthrosis, deep infection, or metallosis. The greatest number of complications were noted in the group with using of the horizontal transclavicular surgical access. Mainly it is the presence of gipostezii in the proximal part of the chest and lateral surface of the shoulder. In the group with vertical miniinvasive access such complications we havent observed. Conclusions We think that the best surgical access in the surgical treatment of fractures of the middle third of the clavicle is a vertical miniinvasive access which allows not only to reduce the number of complications associated with damage of the branches of the supraclavicular nerve, but also to get a better functional and cosmetic results.</p></abstract><trans-abstract xml:lang="ru"><p>Актуальность По данным статей изданных в России переломы ключицы составляют 12,5 – 26,1% случаев среди всех пере- ломов, а в 76,8 – 80,0% случаев перелом локализуется в области диафиза. Количество пациентов получающих оперативное лечение по поводу перелома ключицы в средней трети растет. Параллельно растет и количество осложнений возникаю- щих после хирургического лечения. Цель исследования Изучение количества осложнений, возникающих после открытой репозиции и накостного остеосинте- за переломов ключицы в средней трети при использовании различных хирургических доступов. Материалы и методы Мы обследовали пациентов с переломом ключицы в средней трети, которым выполнялось опера- тивное лечение с использованием горизонтального трансклавикулярного доступа, горизонтального доступа с выделением ветвей надключичного нерва и миниинвазивного вертикального доступа, во время нахождения в стационаре и через 3, 6,12 и 24 месяца, после операции. Результаты и их обсуждение Нами не наблюдалось таких осложнений, как ложный сустав, глубокая инфекция, либо ме- таллоз. Наибольшее количество осложнений отмечалось в группе с использованием горизонтального трансклавикулярного хирургического доступа. Преимущественно это наличие гипостезии в проксимальном отделе грудной клетки и латераль- ной поверхности плеча. В группе с вертикальным минимально инвазивным доступом таких осложнений мы не наблюдали. Выводы Мы считаем, что наиболее оптимальным хирургическим доступом при оперативном лечении переломов ключицы в средней трети, является вертикальный миниинвазивный доступ, позволяющий не только снизить количество осложнений, связанных с повреждением ветвей надключичного нерва, но и получить более лучший функциональный и косметический результаты.</p></trans-abstract><kwd-group xml:lang="en"><kwd>Fracture of the clavicle fixation</kwd><kwd>complications</kwd><kwd>supraclavicular nerve</kwd><kwd>surgical approach</kwd><kwd>minimally invasive osteosynthesis.</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>Перелом ключицы</kwd><kwd>остеосинтез</kwd><kwd>осложнения</kwd><kwd>надключичные нервы</kwd><kwd>хирургический доступ</kwd><kwd>минимально инвазивный остеосинтез.</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
