Несмотря на интенсивную разработку вопросов диагностики и хирургического лечения панкреонекрозов (ПН), летальность остаётся высокой, достигая 30-50% и более. Это связано, с одной стороны, с недостаточной изученностью механизмов панкреатогенной токсемии, генерализации инфекции, проявлений системной воспалительной реакции (СВР) и роли органо-тканевых барьеров в нейтрализации токсинов, а с другой – с неэффективностью лекарственных средств при системном их применении в результате блокировки органного микроциркуляторного русла и ограничения их доступности. В результате исследований нашло подтверждение сложности возникновения СВР и механизмов токсемии при ПН, что проявилось в высоком уровне лейкоцитоза, исследуемых ферментов, особенно при инфицированном ПН, отражающих морфофункциональные нарушения печени, как основного дезинтоксикационного барьера на пути панкреатогенной агрессии. Указанные гуморальные показатели могут служить определёнными маркерами тяжести проявления токсемии и СВР. Применение регионарной артериальной лекарственнойтерапии позволяет достигнуть лучших результатов в снижении летальности при ПН по сравнению с традиционными методами лечения.
