The diagnosis was established using the CEAP basic classification. The research method is ultrasound angioscanning.During the study, a standardized Valsava and Siegel tests were used. In the superficial and deep veins, the diameter and velocityof the antegrade, retrograde blood flow were calculated. In perforating veins (PV), the diameter, maximum and average speed ofretrograde blood flow were calculated.<italic>Results:</italic> With the growth of the clinical class, there is an increase in the diameter of the great saphenous vein (GSV), astatistically significant difference in the diameter of the GSV was obtained between classes C2 and C4, C2 and C5.6, C4 andC5.6. With the growth of the clinical class, there is a gradual increase in the amount of incompetent PV (on average 0.7 per lowerlimb in the clinical class C2 to 1.77 PV in the clinical class C5.6), diameter, maximum and average speeds of retrograde bloodflow through the PV. A statistically significant difference in the diameter of the PV was obtained between classes C2 and C3, C3 and C4, C4 and C5; by the parameter of the average speed of retrograde blood flow through the PV only between classes C2 and C5.6. In addition, a significant difference was determined by the parameters of the maximum and average blood flow velocity forPV Cockett III between classes C4 and C5.6 (P &amp;lt;0.05).<italic>Findings:</italic>1) GSV diameter ≥ 8.9 ± 2.8 mm, PV ≥ 4.3 ± 1.3 mm, as well as the maximum and average retrograde blood flow velocitiesthrough insolvent PV Cockett III ≥ 66.1 ± 28 cm / s and 52 ± 19.7 cm / s are associated with the appearance of trophic ulcers inpatients with VD.2) Insufficiency of the femoral-popliteal segment of deep veins is of minimal importance for the appearance of trophic disordersin VD.<italic>Key words</italic>: varicose disease, hemodynamic disorders, disturbance of microcirculation, trophic ulcers, duplex angioscanning.</p></abstract><trans-abstract xml:lang="ru"><p><italic>Цель исследования:</italic> изучение морфофункциональных изменений венозной системы и патофизиологию венознойгемодинамики по данным дуплексного сканирования у пациентов с варикозной болезнью вен нижних конечностей (ВБ).<italic>Материалы и методы:</italic> В исследование вошли 579 человек с ВБ: 354 пациента с трофическими расстройствами и 225пациентов без трофики. Диагноз устанавливался с помощью базовой классификации CEAP. Метод исследования –ультразвуковое ангиосканирование. Исследование проводилось согласно Российским клиническим рекомендациям подиагностике хронических заболеваний вен от 2018г. При проведении исследования использовались стандартизированнаяпроба Вальсавы и проба Сигела. В поверхностных и глубоких венах вычислялись диаметр, линейная скоростьантеградного, ретроградного кровотока. Для характеристики клапанной недостаточности использовалось отношениеантеградных и ретроградных скоростей. В перфорантных венах (ПВ) вычислялся диаметр, максимальная и средняяскорость ретроградного кровотока.<italic>Результаты:</italic> С ростом клинического класса идёт увеличение диаметра большой подкожной вены (БПВ), статистическизначимая разница в диаметре БПВ была получена между классами С2 и С4, С2 и С5,6, С4 и С5,6. С ростом клиническогокласса идёт постепенное увеличение количества несостоятельных ПВ (в среднем 0,7 ПВ на нижнюю конечность приклиническом классе С2 до 1,77 ПВ при клиническом классе С5,6), диаметра, максимальной и средней скоростиретроградного кровотока через ПВ. Статистически значимая разница в диаметре ПВ была получена между классами С2и С3, С3 и С4, С4 и С5; по параметру средней скорости ретроградного кровотока через ПВ только между классами С2 иС5,6. Кроме того определялась значимая разница по параметрам максимальной и средней скорости кровотока для ПВКоккетт III между классами С4 и С5,6 (P&amp;lt;0,05).<italic>Выводы:</italic>1) Диаметр БПВ ≥ 8,9±2,8 мм, ПВ ≥ 4,3±1,3 мм, а также максимальная и средняя скорости ретроградного кровотокачерез несостоятельные ПВ Коккетт III ≥ 66,1±28 см/с и 52±19,7 см/с ассоциируются с появлением трофических язв упациентов с ВБ.2) Недостаточность бедренно-подколенного сегмента глубоких вен имеет минимальное значение для появлениятрофических расстройств при ВБ.<italic>Ключевые слова:</italic> варикозная болезнь, гемодинамические нарушения, трофические язвы, дуплексное сканирование.</p></trans-abstract><kwd-group xml:lang="en"><kwd>Varicose disease</kwd><kwd>hemodynamic disorders</kwd><kwd>disturbance of microcirculation</kwd><kwd>trophic ulcers</kwd><kwd>duplex angioscanning</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>Варикозная болезнь</kwd><kwd>гемодинамические нарушения</kwd><kwd>трофические язвы</kwd><kwd>дуплексное сканирование</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Савельев В.С.
