Бурденко Минздрава России</copyright-holder><ali:free_to_read xmlns:ali="http://www.niso.org/schemas/ali/1.0/" start_date="2028-04-14"/><license><ali:license_ref xmlns:ali="http://www.niso.org/schemas/ali/1.0/">https://vestnik-surgery.com/journal/about/editorialPolicies</ali:license_ref></license></permissions><self-uri xlink:href="https://vestnik-surgery.com/journal/article/view/1825">https://vestnik-surgery.com/journal/article/view/1825</self-uri><abstract xml:lang="en"><p><bold>Introduction. </bold>The high incidence of blunt liver injuries, general and postoperative mortality necessitate a search for optimal treatment options of the condition.</p> <p><bold>А</bold><bold>im. </bold>To conduct a systematic literature review and meta-analysis and compare various treatment options for patients with blunt liver injury.</p> <p><bold>Methods. </bold>A systematic search for non–randomized studies was conducted from 01 October 2015, a systematic search for randomized studies was conducted without time limits, until February 29, 2024.</p> <p><bold>Results. </bold>There is a clear trend towards non-surgical management of hemodynamically stable or stabilized patients, and in case of hemodynamic instability, the use of liver tamponade followed by angioembolization.</p> <p><bold>Conclusion. </bold>A larger number of well-planned randomized clinical trials are required to concretize the surgical approach to patients with liver injury.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность.</bold> Высокая частота встречаемости закрытых повреждений печени, общая и послеоперационная летальность диктуют необходимость поиска оптимальных вариантов лечения пациентов.</p> <p><bold>Цель.</bold> Провести систематический обзор литературы и мета-анализ по сравнительной оценке различных вариантов лечения пациентов с закрытой травмой печени.</p> <p><bold>Методы.</bold> Систематический поиск нерандомизированных исследований проведен с 01 октября 2015 г., а рандомизированных – без временных ограничений, по 29 февраля 2024 г.</p> <p><bold>Результаты.</bold> Просматривается отчетливая тенденция к неоперативному ведению гемодинамически стабильных или стабилизированных пациентов, а при гемодинамической нестабильности - использование тампонады печени с последующей ангиоэмболизацией.</p> <p><bold>Заключение.</bold> Требуется большее количество хорошо спланированных рандомизированных клинических исследований для конкретизации лечебного подхода к пострадавшим с повреждениями печени.</p></trans-abstract><kwd-group xml:lang="en"><kwd>blunt liver trauma</kwd><kwd>perihepatic packing</kwd><kwd>endovascular surgery</kwd><kwd>surgical treatment of liver injury</kwd><kwd>angioembolization</kwd><kwd>non-operative management</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>закрытая травма печени</kwd><kwd>тампонада печени</kwd><kwd>эндоваскулярная хирургия</kwd><kwd>хирургическое лечение травм печени</kwd><kwd>ангиоэмболизация</kwd><kwd>неоперативное лечение</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Vladimirova ES, Valetova VV.
