Quantitative evaluation of the expression of free radical processes and antioxidant status of the blood system in this pathology is particularly importance. The purpose of the study To determine the condition of free radical oxidation and antioxidant protection in blood serum, to estimate their predictive value, depending on the degree of enteral insufficiency for the postoperative period of patients with acute adhesive intestinal obstruction. Materials and methods 87 patients with acute adhesive intestinal obstruction, treated in a surgical department of SBHF "City Clinical Hospital № 1" in Orenburg in 2007-2012 were examined. All patients were operated emergency. Conservative treatment in the postoperative period in accordance with the current standard diagnostic and treatment was carried. The duration of postoperative intestinal paresis in all patients was estimated. In addition to standard clinical examination the leukocyte index of intoxication and the level of average weight molecules in blood serum was determined. Condition of free radical oxidation evaluated by determining the concentration of malondialdehyde and diene conjugates in blood serum, as well as the study of spontaneous chemiluminescence and iron-induced chemiluminescence of blood serum. Result and their discussion Leukocyte index of intoxication, the concentration of lipid peroxidation products and the average weight molecules in serum in the early postoperative period did not always correspond to the degree of intestinal dysfunction, and it can not predict the duration of postoperative intestinal paresis. Chemiluminescence indicators are informative predictor of enteral insufficiency syndrome in the preoperative and early postoperative period in patients with intestinal obstruction. Conclusion The syndrome of intestinal failure associated with activation of free radical processes and reduced antioxidant status. Indicators of chemiluminescence of blood serum are informative predictor of the development of enteral insufficiency syndrome as in the preoperative and early postoperative period in patients with intestinal obstruction.</p></abstract><trans-abstract xml:lang="ru"><p>Актуальность Острая кишечная непроходимость сопровождается нарушением гемоциркуляции кишечника, развитием ишемии кишечной стенки и энтеральной недостаточности, что приводит к активации свободнорадикальных процессов. В связи с этим большой интерес представляет количественная оценка экспрессии свободнорадикальных процессов, воз- никающих при данной патологии, а так же состояние антиоксидантной системы крови. Цель исследования Изучить состояние процессов свободнорадикального окисления и антиоксидантного статуса сыво- ротки крови, оценить их прогностическое значение в зависимости от степени энтеральной недостаточности в послеопе- рационном периоде у больных с острой спаечной непроходимостью кишечника. Материалы и методы Обследовано 87 больных острой спаечной кишечной непроходимостью, находившихся на лечении в хирургическом стационаре ГБУЗ «Городская клиническая больница № 1» г. Оренбурга в 2007-2012 гг. Всем больным было выполнено оперативное вмешательство в экстренном порядке, проводилось консервативное лечение в послеоперационном периоде согласно действующему лечебно-диагностическому стандарту. У всех больных оценивали длительность после- операционного пареза кишечника. Помимо стандартного клинического обследования определяли лейкоцитарный индекс интоксикации и уровень молекул средней массы сыворотки крови. Состояние свободнорадикального окисления оценивалось путем определения концентрации малонового диальдегида и диеновых коньюгат в сыворотке крови, а также изучения спонтанной хемилюминесценции и железоиндуцированной хемилюминесценции сыворотки крови. Результаты и их обсуждение Установлено, что такие показатели, как лейкоцитарный индекс интоксикации, концен- трация продуктов перекисного окисления липидов и молекул средней массы в сыворотке крови до операции и в раннем послеоперационном периоде не всегда соответствуют степени интестинальной дисфункции и не позволяют прогнозиро- вать длительность послеоперационного пареза кишечника. В тоже время показатели хемилюминесценции наиболее точно соответствуют тяжести энтеральной недостаточности у больных с непроходимостью кишечника в раннем послеопе- рационном периоде. Выводы Синдром недостаточности кишечника сопровождается активацией свободнорадикальных процессов и снижени- ем антиоксидантного статуса.
