Laser Doppler Flowmetry is a non-invasive measuring method of the lower limbs vascular system. The purpose of the study To investigate changes in microcirculatory blood flow and its regulation in patients with atherosclerosis obliterans before and after reconstructive surgery of the aorta-femoral segment. Materials and methods Lower limbs microcirculatory blood flow was detected by percutaneous Laser Doppler Flowmetry with laser Doppler perfusion imager (LАKK – 02, NPО «LAZMA», Russia) in red wavelength spectrum. 54 patients with atherosclerosis obliterans, aorta-femoral segment were investigated after aorta-femoral bypass surgery. Results and their discussion High level occlusion revealed decrease in the amplitude of endothelial functioning range, prevailed neurogenic component of vascular tone and elevated initial value of the bypass index. The rhythmic spectrum of blood flow did not reach its baseline level in cuff-based blood flow occlusion after effective revascularization. The high level of bypass index after aorta-femoral reconstruction was related to arteriola-venular shunt. Conclusion Patients with high level occlusion required additional microvasculature assessment in the preoperative period and further correction after reconstructive surgery taking into account the pathophysiological mechanisms regulating the processes of microcirculation</p></abstract><trans-abstract xml:lang="ru"><p>Актуальность Изучение эффективности реконструктивных операций при облитерирующем атеросклерозе невозмож- но без объективного контроля за состоянием не только магистральных сосудов, коллатерального кровообращения, но и микроциркуляторного русла нижних конечностей. Лазерная допплеровская флоуметрия является не инвазивным методом оценки сосудистого бассейна нижних конечностей. Цель исследования Изучение функционального состояния микроциркуляторного русла и систем его регуляции у больных облитерирующим атеросклерозом артерий нижних конечностей до и после реконструктивных вмешательств на аорто- бедренном артериальном сегменте. Материалы и методы Функциональное состояние микроциркуляторного русла нижних конечностей изучали методом чрескожной лазерной допплеровской флоуметрии лазерным анализатором капиллярного кровотока (ЛАКК – 02, НПО «ЛАЗМА», Россия) с использованием базового светового зонда в красном канале измерения у 54 пациентов облитерирую- щим атеросклерозом артерий нижних конечностей при поражении на уровне аорто-бедренного артериального сегмента и в динамике после проведения аорто-бедренного шунтирования. Результаты и их обсуждение При высоком уровне окклюзии выявлено снижение амплитуды в диапазоне эндотелиальной активности, доминирование нейрогенного компонента сосудистого тонуса, исходно высокое значение показателя шун- тирования. Обнаружено отсутствие нормализации ритмического спектра кровотока на окклюзионную пробу на фоне эффективной реваскуляризации конечности. Высокий уровень показателя шунтирования после аорто-бедренной рекон- струкции свидетельствует о артериоло-венулярном сбросе.
