The need to replace the patella in total knee arthroplasty is still a subject of debate among orthopedic surgeons. Existingstudies show heterogeneous and even contradictory results. А study of 249 patients with unilateral knee osteoarthrosis , whounderwent total knee arthroplasty with (group I) and without (group II) patellar resurfacing in the orthopaedic departmentof National Medical Surgical Center named N.I.Pirogow was carried out. Group I included 128 patients (51.4%), and groupII included 121 patients (48,6%), Posterior Stabilized Sigma (DePuy Johnson&Johnson) implants were used. We studiedthe rate and structure of general and specifi c (patellofemoral), complications and the revision rate related to patellofemoralcomplications after total knee replacement with and without patellar resurfacing. On the basis of the study results, weconcluded that, with the design of prosthesis used in our study, in patients with primary knee osteoarthrosis, similar resultsmay be achieved with and without patellar resurfacing.

Необходимость замещения суставной поверхности надколенника при тотальном эндопротезировании коленного су-става до сих пор является предметом дискуссий среди ортопедов. Имеющиеся исследования показывают неоднород-ные и даже противоречивые результаты. Было выполнено исследование 249 больных с односторонним гонартрозом,перенесших операцию по тотальному эндопротезированию коленного сустава с замещением (I группа) и без заме-щения (II группа) суставной поверхности надколенника в отделении травматологии и ортопедии НМХЦ им. Н.И.Пирогова. В I группу вошло 128 пациентов (51,4%), а во II группу вошел 121 пациент (48,6%). Изучались частотаи структура общих и специфичных (пателлофеморальных) осложнений, а также частота ревизионных операций поповоду пателлофеморальных осложнений. На основании результатов исследования, с использованием эндопротезовданной модели у пациентов с первичным гонартрозом, можно получить одинаковые результаты как с замещениемсуставной поверхности надколенника, так и без него.
