Only in our clinic, the proportion of patients with destructive forms of erysipelas is about 18% of allpatients of the Department of purulent surgery.The aim of the present investigation is to improve the results of treatment complications associated with erysipelas according tomodern concepts about this problem.Methods We conducted a randomized controlled trial of 34 patients suffer from erysipelas, associated with purulent complications.All patients were categorized according to kind of treatment into two groups. The control group (17 patients) included patients treatedwith conventional therapy. In the main group (17 patients) in the conventional treatment of erysipelas was included immunomodulatorDerinat. The efficacy of the treatment was monitored through clinical tests with calculation of indexes of intoxication (LII, YII), andspecial studies of the immune status.Results In patients treated with Derinat symptoms of intoxication were stopped in earlier periods (6 ± 0,58 (p&lt;0,5) day, LII and YIIwas normalized on the 7th day) in comparison with the control group. After the traditional therapy, in patients was the suppressionof T-lymphocytes and their subpopulations, ingestion rate was lower than in healthy persons. In the main group of patients aftertreatment with derinat we are reveal the stimulation of T - and B - immunity with increased ingestion rate.Discussion The inclusion of Derinat in the conventional therapy of patients with erysipelas allows to correct disorders in the immunestatus of these patients, which contributes to their rapid healing and reducing the risk of recurrence of the disease.Key words: erysipelas, destructive forms of erysipelas, complications of erysipelas, Derinat.</p></abstract><trans-abstract xml:lang="ru"><p>Актуальность Интерес хирургов к рожистому воспалению (РВ) обусловлен все возрастающим количеством пациентов сгнойно-некротическими осложнениями данного заболевания. Только в нашей клинике доля пациентов с деструктивнымиформами рожистого воспаления составляет около 18% всех пациентов отделения гнойной хирургии.Целью исследования явилось повышение эффективности медикаментозной терапии деструктивных форм рожистоговоспаления (РВ) с учетом современных представлений об этиологии и патогенезе заболевания.Материалы и методы Анализированы результаты лечения 34 пациентов с деструктивными формами РВ. Группа сравне-ния (17 пациентов) получала традиционную для РВ терапию. В основной группе (17 пациентов) в общепринятую схему ле-чения РВ был включен иммуномодулятор деринат. Эффективность проводимого лечения контролировалась посредствомобщеклинических анализов с расчетом индексов интоксикации (ЛИИ, ЯИ), а также специального исследования иммунногостатуса.Результаты и их обсуждение Назначение дерината больным РВ позволило купировать общеинтоксикационный синдромв более ранние сроки, в сравнении с традиционным лечением (на 6 ± 0,58 (р&lt;0,5) сутки, ЯИ и ЛИИ нормализовались на 7-есутки). После проведения традиционной терапии у пациентов сохранялась супрессия Т-лимфоцитов и их субпопуляций,фагоцитарная активность была ниже, чем у здоровых лиц.
