Optimization of semi-closed method of treatment of the diffused peritonitis

Abstract

There was done complex analysis of 180 patients with the diffused purulent peritonitis, which were divided into two groups. The first (control) group included 131 patients treated by the traditional technology. The second (basic) group included 49 patients, treatment of which was done be the same technique was except the fact that before suturing of the abdominal cavity instead of the water solution there was introduced 200 ml of the immobilized form of sodium hypochlorite  in 5% gel of the carboxymetillcellulose. Before the surgical intervention severity of patients’ condition was determined by APACHE II scale, the level of endogenic intoxication and Mangham’s index of peritonitis. Efficacy of treatment was assessed by dynamics of indicators of the systemic endotoxicosis, motoric-evacuating function of the intestines, frequency of complications and lethality. Original technology of treatment enables to improve motoric-evacuating function of the intestines in 2 times,  to decrease the indicators of endotoxicosis in 1,6 times, number of complications – in 2 times, post-operative lethality – in 2,2 times.
