Correction of lipid-transport system at experimental widespread purulent peritonitis

Abstract

Research is spent on 40 rabbits-males of chinchilla breed. For the first time influence of a preparation of omega-3 fatty acids «Omegaven» on protein-lipid spectrum of blood is studied at an experimental widespread purulent peritonitis. It is established that in the postoperative period «Omegaven» effectively influences on protein-lipid spectrum of blood by prevention of sharp decrease in quantity of proteins, lipoproteins of high density (HDL) and maintenance of triglyceride’s growth. Positive action of the preparation also is connected with percentage decrease of lisophosphatides and level increase of polyglycerophosphatides in phopholipid spectrum of HDL.

Dynamics and surgical correction of multiple organ failure in patients with general purulent peritonitis with severe initial manifestations of systemic inflammation

The article presents the results of the surgical treatment of 70 patients with general purulent peritonitis with severe initial manifestations of systemic inflammation. We studied the effect of permanent transmembrane peritoneal dialysis in the intraand postoperative period on the dynamics of multiple organ failure, and validated the effectiveness of this method in patients with general purulent peritonitis.

Morphological aspects of the influence of antiorthostatic hypokinesia on the experimental peritonitis

Abstract

The purpose of this study was to reproduce peritonitis in simulating the effects of weightlessness and to identify the morphological features of its development. Methods. Eighty white rats were divided into 4 groups: (1) vivarium control, (2) antiorthostatic hypokinesia (AH), (3) peritonitis alone, and (4) AH with peritonitis. Effects of AH were achieved by putting rats on a special stand for a period of 14 days, followed by the formation of peritonitis. Peritonitis was induced by laparotomy, cross-section of the colon length of 0.5 cm followed by suturing laparotomic wound. Twenty-four hours after the operation, the rats were killed by decapitation; and their liver and pancreas have been investigated. The most noticeable morphological changes in the liver during AH and peritonitis were circulation disorders - a minor under AH, and rough with peritonitis. AH causes congestion and excessive accumulation of zymogen in the pancreatic cells. Peritonitis leads to disruption of the structural design of zymogen. The combination of AH and peritonitis leads to the fact that autolysis of pancreatic cells caused by peritonitis amplified with a high content of zymogen caused by AH and implemented in the common pancreonecrosis. Morphological analysis revealed that in microgravity the destructive changes caused by peritonitis in liver decrease and increase in the pancreas.

The assesement of blood aggregation in the treatement of choleperitonitis with sodium hypochlorite

Abstract

Animals with choleperitonitis have a growth of erythrocytes stimulatory effect on platelets aggregation and sectetion and vice versa, accompanied by the increase of hematokrit and the decrease of erythrocytes deformability. In the treatment of animals in the control group the maximum decrease of MDA occurred at day 7, while in the experimental group – at day 3. The study of blood antiaggregatory properties in the suspension of platelets with thrombin on animals of control group and experimental group at days 1, 3 and 7 with respect to the samples of the platelet suspension in intact animals without thrombin shows the higher degree of blood aggregation inhibition on animals of experimental group. Such a mechanism suggests that the oxidative effect of sodium hypochlorite is not simply inactivate of thrombin receptor and loss of the thrombin effect. It is possible that, on the one hand, the suppression of platelets aggregation and, on the other hand, the effect of cyclooxygenase inhibition by sodium hypochlorite are caused by different types of modification of the platelets plasmatic membrane.

The method of treatment of acute destructive cholecystitis patients in elderly

Abstract

With the years there is a structural both functional disadaptation and the organization of systems an organism. The great
influence on the given process renders number of accompanying diseases. Their cumulative effect reaches a maximum at
advanced age, doing traditional methods of surgical treatment by the extremely risky. For an estimation of operational risk
classification МНОАР(1989) with definition of degree of risk during all periods of surgical treatment is modified: presurgical,
intraoperative and postoperative. Calculation of indicators of risk of 111 patients at the age from 45 till 89 years which have
been hospitalized in a surgical hospital concerning an acute calculous destructive cholecystitis is made. By the given patient it
was spent two principal views of interventions: a traditional cholecystectomia and maloinvazive technologies. The age doesn't
render authentic influence on differences of operational-anesthesiology risk. Performance of a traditional cholecystectomia
is bound to high operational-anesthesiology risk during an operational intervention. Application maloinvazive methods of
treatment of an acute calculous cholecystitis allows to lower risk and to improve results of treatment

Application miniinvaziv technologies in treatment of patients with the complicated sharp cholecystitis

Abstract

Laporoscopic cholecystectomy was condacted in 2467 patients with acute cholecytitis. All complications was manged by
miniinvasiv technologies. A convertion observed in 11 (0,4%) cases, mortality stands at 1,1%.With this we can make a
conclusion that at this modern stage of endosurgery a laporoscopic surgey becomes a treatment of choice for management
of an acute cholecytitis.

Assessment of function of external breath and bronchoscopic picture in patients with calculous cholecystitis and features of their dynamics in the postoperative period

Abstract

Assessed the function of external breath and state value of the tracheobronchial tree in 50 patients with cholelithiasis
on the basis of the results of spirometry and bronchoscopy. Researches were performed before and 3-5 days
after cholecystectomy performed by mini - invasive techniques. Infringements of function of external breath were detected
before operation in 42 (84, 0 %) patients, thus prevailed restrictive disorders - 97, 6 %. Changes in the tracheobronchial
tree were found in 39 (78.0%) patients. In the early postoperative period observed negative dynamics of parameters of
function of external breath and bronchoscopic picture. The specified changes were characterized by an increase of frequency
of revealing "active" endobronchitis from 66, 0 % to 86, 5 % (р=0, 05) and increase of obstructive disorders of function of
external breath from 52, 0 % to 74, 0 % (р=0, 04) which development is not caused by a painful syndrome and restriction of
diaphragm mobility

The change microcirculation hemostasis in patients with the peritonitis on a background the arterial hypertension

Abstract

The aim was to study of microcirculation at 17 patients with peritonitis on arterial hypertension in the early postoperative
period and 15 patients with peritonitis without concomitant diseases. The concentration of ADAMTS - 13 and its inhibitor,
the level of TFPI were determined by ELISA. We studied the spontaneous and induced platelet aggregation, as well as the
definition of LTA and the degree of adgesion. In patients with peritonitis on a backround of arterial hypertension increased
rate LTA in 2,5 fold, been an increase in the degree of spontaneous aggregation in 1,6 fold and a significant increase in the
degree and rate of aggregation induced by epinephrine in 1,9 fold with respect to a clinical comparison. The level ADAMTS-13
is increased fold 1,4 of аt patients with peritonitis on a background of hypertension, the levels of TFPI increased fold 2
compared with the control group. It was established, that in patients with peritonitis on arterial hypertension has rheological
imbalance manifested revised indicators of microcirculation and hemostasis. During systemic inflammation and arterial
hypertension potentiated the deviation of the functional state of endothelial cells

Modern semiotics of acute adhesive intestinal obstruction

Abstract

Acute adhesive intestinal obstruction is the most serious late postoperative complication. Postoperative mortality in violation
of passage of intestinal contents, caused by adhesions formed as high as 15%. The structure of urgent surgical pathology
adhesive ileus is a leader. According to the literature, the implementation of minimally invasive interventions, the perfection
of suture material and so should reduce the number of patients with this postoperative complication. The paper presents an
analysis of patient diagnosed with acute intestinal obstruction admitted to the surgical department of Belgorod City Clinical
Hospital. Studied medical history, condition at admission, clinical manifestations of acute adhesive intestinal obstruction,
evaluated the diagnostic possibilities of instrumental methods of examination. Shows the dynamics of referral of patients for
the years analyzed, as well as the tactics of treatment and outcome of disease.

Bioimpedansometrical and morphological analysis of zone of intestinal anastomoses, imposed after small bowel resection during experimental acute intestinal obstruction

Abstract

It is presented an analysis of changes in impedance indices of zone of intestinal anastomoses in 10-14 days after resection
of the small intestine at different distances from the zone of strangulation during experimental acute intestinal obstruction,
compared with the norm. It is shown that histomorphological changes of the reparation of anastomoses, imposed after
resection of intestine in acute intestinal obstruction based on indicators of impedance, and anastomoses performed after
resection of the intestine within a clearly viable tissue are not significantly different

Clinico-morfologic signs of the complications of the ulcer disease with chronic duodenal obstruction

Abstract

The results of the treatment of 118 patients with chronic complications of the ulcer disease were studied. 1 group- 60 patients
operated with chronic complications of the ulcer disease without correction of the motility of the duodenum. 2 group – 58
patients operated with chronic complications of the ulcer disease and correction of the chronic duodenal obstruction was
done. Morphologic investigation of the stomach and duodenal mucous membrane showed that after conservative or surgical
correction of the chronic duodenal obstruction in patients with chronic complications of the ulcer disease signs of disturbed
secretion, protection function and endocrinopathy were removed.

The results of endoscopic ultrasound artificial esophagus, formed from the left half of the colon. The data on the structure
of endosonographic coloanastomos. Three types of blood supply to the graft and esophagocoloanastomos (trunk, loose and
mixed). We prove that the main type of blood supply is prognostically important in case of stenosis of the anastomosis.

Change of some coagulation-lytic system of blood in the ischemic area of the small intestine after intravenous, subcutaneous and intraoperative paravasal routes of administration of heparin in the experiment

Abstract

The aim of the study was a comparative evaluation of the effectiveness of distinct prevention of abdominal adhesions after
intravenous, subcutaneous and intraoperative paravasal routes of administration of heparin in conditions of ischemic injury
of the small intestine. The paper presents an experimental study conducted on 40 adult mongrel dogs were divided into 4
groups (10 dogs in each group). The first group formed the stomach in kind, which was not carried out heparin, the second
- the animals, which the eye was performed, intravenous heparin, and the third - the animals, which produced subcutaneous
heparin and the fourth - of animals, which was carried out intraoperative heparin in the tissue of a neurovascular beam
mesentery. From the venous vessel, directly adjacent to the ischemic area of the small intestine 20 minutes after creating
the model for the fracture circulatory organs are made to draw blood with subsequent study of several coagulation-analytic
system of blood: clotting time by Lee-White, prothrombin time and prothrombin ratio, the activity of antithrombin III.
After taking the blood from the venous vessel last tied. Then, the surgical wound sutured in layers. Postoperatively, any
treatment of animals was conducted. At various times after surgery were evaluated functional and morphological changes in
the experimental section of the intestine. Based on the data shown conclusive advantages of intraoperative paravasal the set
route of administration of heparin in the prevention of abdominal adhesions.

Influence of heptral on the condition of the liver tissue after its resection

Abstract

In experiments on 37 rabbits studied the influence of a controlled ischemia of the liver on its morphology and function when
administered heptral, belonging to the pharmacological group hepatoprotectors. Three series of experiments with ligation
hepatoduodenal ligament for 20 and 30 minutes and resection of the left outer lobe of the liver. It is shown that operation
without the introduction of heptral are accompanied by increased content of lipid peroxidation (LPO) in liver tissue at the
same time there are pronounced structural changes. Established that heptral reduces the severity of structural and functional
changes in the liver in conditions of prolonged ischemia body up to 30 minutes, has antioxidant properties, helping to reduce
the intensity of the processes LPO

The use of ozonized Perftoran in treating of purulent wounds

Abstract

On a base analysis of results treatment 118 patients with acute pyoinflammatory diseases of skin and subcutaneous soft
tissues is fulfilled comparison of three modes treatment primary purulent wounds: with Perftoran, by traditional way and
with ozonized Perftoran. It were studied physic-chemical features of Perftoran administered for local application before and
after ozonizing. The revealed changes dispersed composition Perftoran not led to a decrease in efficiency in the use of this
drug. It was found out that treating purulent wounds with Perftoran and combination Perftoran and ozone have advantages
over traditional way of treatment and clinical effectiveness ozonized Perftoran is higher than clean preparation. Using for
purulent wound treatment ozonized Perftoran resulted in notable decreasing of patient’s treatment duration

The development of circular venous leg ulcers in patients with chronic venous insufficiency leads to the formation of chronic
compartment syndrome. However, the symptoms often go unnoticed, which leads to poor functional outcome. In the article
the principles of diagnosis compartment syndrome of the lower extremities with CT in 58 patients C6 CEAP class. All studies were performed on multislice computed tomography «Aquillion» company «Toshiba». Conducting СТassessment of
qualitative and quantitative characteristics of the state and the volume of tissue compartments of leg, especially indicators
of muscle density, makes it highly reliable to detect the presence and severity of chronic compartment syndrome in patients
with chronic venous insufficiency C6 class

Ways to improve treatment of epithelzed pilonidal sinun

Abstract

In structure of proctological diseases the epithelzed pilonidal sinus (EP) took fourth place. Choosing the wrong medical
tactics and method of operation lead to complications and associated diseases in 6-30%, suppuration and diseases in chronic
form of the 10-30% operated. To improve the results of surgical treatment in patients with different stages of disease EP
proposed the differential approach to a choice of method of treatment. Under supervision were 230 patient at the age from
17 to 53 years. The control group (I) was made by 102 patient operated by a traditional technique. The basic group included
128 patients. In this group the method of a choice of operative treatment and a variant of closing of a wound depended on
prevalence of inflammatory process and topografo-anatomical fiatures cluneal-coccygeal area. Wound closing was carried
out intracutaneous sutures. In the first group of 19 (18.6%) patients had postoperative complications. In the second group the
complicated current was marked only at 6 (4.7%) patients. Disease relapses have arisen at 11 (10.8%) patients control group
and 1(0.8%) patient of the basic group

Prognosis of complications by infrared thermography in treatment of children with facial bites

Abstract

Background. The treatment of facial wounds after dog bites in children is a problem of surgery. Reconstructive and plastic
operations in damaged tissues are associated with high risk of complications. The prognosis of reparative process in area
of operation in postoperative period is discussed question. Infrared thermography is known as a way of monitoring after
surgical procedure but importance of this method for prognosis of complications in urgent facial surgery in children at present
is not estimated. Methods. In 2010-2011 in the Department of Maxillofacial Surgery (Nizhny Novgorod Regional Children
Hospital) the prospective controlled study of treatment facial bites in children is carried out (n = 33). By all patients we have
undertaken standard surgical procedures to complete correction of facial injuries. In first group were included all patients
with a smooth postoperative period (n = 22), in two group - those who developed complications (n = 11). In postoperative
period we have used the infrared thermography (CEM ThermoDiagnostics) and compared data by Kolmogorov-Smirnov.
When thermographic differences were no more than 1,5° C, a smooth postoperative period was observed. When the
temperature was higher in zone of operations more than 1,5° in comparison to contrlateral area, we have noted inflammatory
complications. In cases with the local temperature low than 1,5° in comparison to intact zone were observed necrosis of skin
flap. Distinction of thermographic results between analyzed groups was significant: p=0,0000001, Z=0,35249, D=0,95455
(Kolmogorov-Smirnov). Reliable correlation between thermographic changes and complications development was found:
r=0,816, p=0,0000001 (Spearmen). Local hypothermia more than 1,5° C was associated with formation of skin flap necrosis.
Increased local temperature more than 1,5° C was associated with the development of inflammatory complications. The use
of thermography can predict possible variants of postoperative period

The immune oriented nutrition support for patients with severe acute nontraumatic intracranial hemorrhage

Abstract

The actual work presents the results of using various programs of nutrition support for patients with severe nontraumatic
intracranial hemorrhage. The huge reduction of both cellular and humoral immunity is established in the first day of the
onset of the disease. The authors assess the impact of the combined enteral-parenteral nutritive support with dipeptide
alanine-glutamine on immune rates and severity of nutrition deficiency. The work deals with the comparative characteristics
of complications of these nutrition programs. It is established, that application of alanine-glutamine in complex intensive
therapy reduces the frequency of pneumonia, delays pneumonia development, reduces the duration of mechanical ventilation
and the time of ICU stay.

The effect of mechanoactivated calcium gluconate and polarized polychromatic light on the reparative processes in the bone tissue of the mandible

Abstract

There was an experimental investigation of the effectiveness of mechanoactivated calcium gluconate (MACG) and polarized
polychromatic light “Bioptron” on reparative processes of the bone tissue in mandible fractures. The experimental
investigation was carried out on 63 male rabbits divided into three groups. The first group was control, in the second group of
rabbits the postoperative wounds were irradiated with “Bioptron”, in the third group rabbits were given mechanoactivated
calcium gluconate with food and their wounds were irradiated with polarized polychromatic light “Bioptron”. The experiment
proved the effectiveness of the complex of peroral administration of MACG and irradiation of postoperative wounds with
polarized polychromatic light due to the acceleration of the reparative processes in bone tissue of the mandible

Damage of a spinal cord at children has the features connected with a greater extensibility of a backbone concerning a spinal
cord. In the English-speaking literature traumatic traction myelopathy names the syndrome SCIWORA. We have done the
experimental model traumatic traction myelopathy at experimental animals with a various degree of neurologic deficiency.
All cases were accompanied by different electromyography symptoms and characterized by fast recourse of neurologic
deficiency. We have revealed discrepancy of clinical and morphological changes. Presence of subarachnoidal haemorrhages,

topography and gistotopography of the prostate gland. The study was carried out of prostatoptosis the first and the second
degree in the process of treating the symptoms of the violation of urination “of per rectum” massage of the prostate gland
and physiotherapy exercises, directed on the joints and bone-muscular machines belt of the lower extremity. The suggestion
was made on the introduction into the list of diseases of the prostate gland this nosology as “prostatoptosis— descent of the

Segmental lymph stimulating therapy in postoperative period in patients with chronic lower extremities ischemia

Abstract

The analysis of postoperative treatment of 80 patients with chronic lower extremities ischemia (leriche syndrome). The fi rst (control) group includes 20 patients with 2b stage of chronic ischemia of lower extremities without acute ischemia who underwent surgical treatment and standard postoperative treatment. The second (study) group includes 20 patients with the same stage of ischemia who underwent standard postoperative treatment complemented with segmental lymph stimulating injections. Another two groups has the same separation in postoperative treatment and quantity, but patients has critical lower extremities ischemia (the 3rd and the 4th groups). The complications frequency such as oedema and lymphorrhea (reperfusion syndrome) was lower in the 2nd group (p>0,05). Hospitalization term was less in the 2nd group. There were no lymph stimulating injections infl uence on the ankle-brachial index dynamic in postoperative period. In the 2nd and the 4th group pain intensity was diminished.
